What Does this MRI Scan Result Mean?

just had a mri and the results said that There are minimal subcortical white matter T2/Flair hyperintensities, which are non specific but most likely represent a chronic microangiopathic change. I have been having migrains, trouble sleeping, trouble remembering things and difficulty finding words when I speak.

Hello and welcome to HCM,
The symptoms described by you - migraine, trouble sleeping, trouble remembering words, difficulty finding words are neurological symptoms i.e. pertaining to brain.
The MRI report suggests a change in the white mater which represents a small vessel disorder called 'microangiopathic change'.
A cause for this particular change needs to be found out to treat the condition.
A number of cases of microangiopathic change are auto-immune in nature and responds to immune-modulators.
I suggest you to consult a neurologist for clinical assessment, relevant investigations and thus management.
